Privacy Policy

The Comprehensive Primary Health Services - Non-Communicable Diseases / CPHC – NCD application is designed, developed and maintained by Ministry of Health and Family Welfare (MOHFW), Government of India. This application is developed to be used by the state health institutions / functionaries (service providers at public health facilities) to support service delivery at the community level and public health facilities level.

Information Collection and Use

The application captures personal and health information of service users (individuals seeking services from public health system) through mobile application or portal. It also captures your personally identifiable information like your name, mobile number, user name, password and other registration information.

The data will be processed only for the purpose of service delivery, planning, monitoring and research by the State and National health departments.

The information collected by the CPHC- NCD application is stored securely in the Central/ State Data Centre managed by Ministry of Electronics and Information Technology (MeitY) empanelled GCC partners under direct supervision of Central / State authorities.

The application is meant for use by the designated users/health functionaries only. Misuse by non-targeted users should be prevented by owner of the mobile/ tablets/ laptops/desktops.

To improve service delivery, SMS will be sent to users. We take adequate security measures to protect the information collected through the application. Any information provided to CPHC- NCD application will be protected from loss, misuse, unauthorized access or disclosure, alteration, or destruction as per Government of India policies.

We maintain the data privacy and do not sell or share any personally identifiable or health information captured through the application by field health functionaries of the public health facilities to any third party (public/private).

Log Data/Automatically Collected Information

In case an error occurs while using the application, we collect data and information (through third party products) on your phone called Log Data. This Log Data may include information such as your device Internet Protocol (“IP”) address, device name, operating system version, the configuration of the app when utilizing our Service, the time and date of your use of the Service, and other statistics to improve the service, to debug and provide bug fixes.

We would also like to inform you that the location of user will be captured with GPS technology. If the user does not provide permission, application will not collect the data.

To understand, improve user experience and to develop new features, the software will track application features usage by the users and will send consolidated data on need basis to server.

Data Retention Policy, Managing Your Information

We will retain user data as well as automatically Collected information in the server if you use the application. The information will not be visible once the user is marked as inactive.

We may disclose User provided and Automatically Collected Information as required by law, such as to comply with a subpoena, or similar legal process when we believe in good faith that disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud, or respond to a government request with our trusted service providers who work on our behalf, do not have an independent use of the information we disclose to them, and have agreed to adhere to the rules set forth in this privacy statement

This application adhere to all guideline issued by Ministry of Electronics and Information Technology (MeitY) on the said subject.

Though all efforts have been made to ensure the accuracy and currency of the content on this application, the same should not be construed as a statement of law or used for any legal purposes. In case of any ambiguity or doubts, users of application (service providers at public health facilities) are advised to verify/check with the Ministry of Health and Family Welfare to obtain appropriate professional advice.

Under no circumstances Ministry of Health and Family Welfare (MOHFW) will be liable for any expense, loss or damage including, without limitation, indirect or consequential loss or damage, or any expense, loss or damage whatsoever arising from use, or loss of use, of data, arising out of or in connection with the use of this application.

These terms and conditions for the application shall be governed by and construed in accordance with the Indian Laws. Any dispute arising under these terms and conditions shall be subject to the jurisdiction of the courts of India.

As the user of the application, you must comply with both the application terms and condition and Service terms and conditions as these apply to the use of services that you access and use through this application. Any violation of these Application terms and conditions or the Service terms and conditions may result in the termination of your access to the Application and/or the Service.

CPHC-NCD App is for your own personal use only. Any commercial use will result in termination of your access to the app and service. You cannot distribute or copy or modify any part of it in any way. You must not attempt to extract any source code from the Application, disassemble it or make any derivative versions, or attempt to interrupt or decipher the transmissions between the Application and our (MOHFW) systems.

Ministry of Health and Family Welfare does not object to the users of CPHC-NCD application linking directly to the information that is hosted on its site, however, an express written consent for displaying any material on this hyperlink shall remain with the Ministry of Health and Family Welfare.

Material featured on this Portal may be reproduced free of charge after taking proper permission by sending a mail to us. However, the material has to be reproduced accurately and not to be used in a derogatory manner or in a misleading context. Wherever the material is being published or issued to others, the source must be prominently acknowledged. However, the permission to reproduce this material shall not extend to any material which is identified as being copyright of a third party. Authorisation to reproduce such material must be obtained from the departments/copyright holders concerned.

As the user of the Application, you must not use the app for any the following

  • You must not use the App in a way that may damage or impair the App, the Service or the underlying systems and security.

CPHC- NCD Application and all copyright, database rights, and other intellectual property rights related to it belongs to Ministry of Health and Family Welfare (MOHFW).
